Responsibilities

• Serves as the primary point of contact to the parent or guardian of the child when they are referred for evaluation and assessment.
• Informs families of their rights and procedural safeguards.
• Facilitates and participates in developing, reviewing, and evaluating Individualized Family Service Plans (IFSPs).
• Coordinates early intervention services and other services that the child may need or may receive, including evaluations and assessments.
• Provides ongoing service coordination for families that may include developing the initial IFSP, ensuring that early intervention services are being implemented according to the IFSP, conducting IFSP reviews, coordinating providers, and identifying resources for families.
• Coordinates the transition process for families, including scheduling and participating in meetings and coordinating with the VI Department of Education and the Head Start program when requested.
• Ensures services are provided on time in natural environment settings such as homes, childcare centers, libraries, and other community settings.
• Completes required documentation, including intake forms, IFSPs, service request forms, referral forms, statistics, etc.
• Submits monthly reports on program objectives and work plans.
• Participates in community-based outreach activities.
• Liaises with all external independent contractors and healthcare providers relative to the program initiatives.
• Offers input for program specific guidelines and standard operating procedures.
• Performs other related duties as required.

DEFINITION

Under the program administration of the Director of Infants & Toddlers or designee, the Infants & Toddlers Service Coordinator supports children with developmental disabilities and their families to obtain services to fulfil their life goals. The Infants & Toddlers Service Coordinator ensures that the family’s and child’s rights are protected by law as mandated by the Individuals with Disabilities Education Act (IDEA), the primary federal law governing the education of students with disabilities. This position requires a confidential relationship to the policymaker and is privy to highly sensitive confidential information.

DUTIES (NOT ALL INCLUSIVE)

Serves as the primary point of contact to the parent or guardian of the child when they are referred for evaluation and assessment.

Informs families of their rights and procedural safeguards.

Bachelor's degree from an accredited college or university in an allied health profession, social work, psychology, or education and at least two (2) years of professional experience in relevant community-based service provision.

OR

An associate degree from an accredited college or university in an allied health profession, social work, psychology, or education and at least four (4) years of professional experience in relevant community-based service provision.

OR

High School Diploma from an accredited institution or its equivalent and six (6) years of professional work experience working with families and children with disabilities.

FACTOR 1- KNOWLEDGE REQUIRED BY THE POSITION

FACTOR 2- SUPERVISORY CONTROLS

Immediate supervision is received from the Program Director, with collaboration by the Program Administrator and Case Management Planner. The Infants & Toddlers Service Coordinator is expected to plan and carry out work independently, drawing upon collaborative support when necessary. Work is reviewed for productivity and adherence to program objectives, guidelines, and work plans.

FACTOR 3- GUIDELINES

Guidelines consist of program, departmental and federal policies, procedures, and instructions. The incumbent adheres to partnering agencies' policies and procedures when working in collaboration with them to provide services.

FACTOR 4- COMPLEXITY

Work involves coordinating the provision of early intervention services and other related services. The Infants & Toddlers Service Coordinator must effectively interact with various agencies in the community. The incumbent may be party and privy to high confidential information to support the requirements of the program and/or the department.

FACTOR 5- SCOPE AND EFFECT

The purpose of work is to enhance families' capacity to meet their children's unique needs with developmental delays by providing timely family-centered, multidisciplinary early intervention services in their natural environment and native language to the best extent possible.

FACTOR 6- PERSONAL CONTACTS

Contacts are co-workers, inter‑departmental partners, representatives from local partner organizations, healthcare providers, families of actual and potential program clients, and community members.

FACTOR 7- PURPOSE OF CONTACTS

Contacts are made to assist the families in obtaining access to other services outside of the program when necessary.

FACTOR 8- PHYSICAL DEMANDS

Work is usually sedentary with light activity including local and off‑island travel, transport of light equipment and materials, and standing for several hours at a time. Occasional work on weekends or evenings maybe required.

FACTOR 9- WORK ENVIRONMENT

Work is in an office‑setting with planned activities in community‑based locations, such as homes, day‑care centers, and public events.
