Job summary

Community Action Agency in Michigan seeks an Early Head Start case manager to provide weekly home visits, assess infant/toddler development, and support families through the family partnership process. You will refer families to CAA programs and community services to meet goals and promote school readiness.

The role requires strong observation, documentation, and collaboration with health and education professionals, with a focus on building trusting relationships with diverse families.

Qualifications

• Bachelor’s degree in social work or related human service field is required.
• Master’s degree in field is preferred.
• Two (2) years’ experience in a related field is preferred.
• Active state licensing in designated field is preferred.
• Infant Mental Health training preferred.

Responsibilities

• Provide weekly home visits to eligible families with infants and toddlers.
• Promote family engagement and referral to CAA and community programs.
• Maintain a caseload and provide case management using the Infant Mental Health model.
• Recruit to maintain full enrollment for Early Head Start.
• Collaborate with inter-agency teams and community partners to support outcomes.
• Document all family contacts and maintain records in data systems.
• Organize parent training activities and support groups.
• Prepare families for preschool readiness and transition planning.
