Job description

Individualized Home Support (IHS) Specialist/ ILS (FT)

Individualized Home Support (IHS) is designed to help people with mental disabilities, mental health conditions, or chronic illnesses to gain independence and self-sufficiency within the community. IHS workers are caregivers that are able to teach individuals life skills, problem-solving, and coping skills to enrich the quality of life of individuals with disabilities to live independently in the community.

This may include but is not limited to:
  1. Safety skills in the community and home
  2. Social skills enhancing relationships both personal and business
  3. Integrating into the community
  4. Assisting with paperwork and appointments
  5. Finding and accessing community resources (food shelves, housing, employment, transportation)
  6. Budgeting and financial management (bill paying, balancing accounts)
  7. Household management (grocery shopping, meal planning/prep)
  8. Organizing (items, areas in home, paperwork)

IHS workers must be able to implement support plans created by client's teams, keep accurate documentation of services provided, and be familiar with 245D paperwork required by the State of Minnesota.

IHS workers must use their own personal vehicles for transporting clients within the community. Most clients are ambulatory, few may use walkers/wheelchairs. (Will fit in the back seat) Mileage reimbursement is provided.

Requirements for this position:
  • Must pass the Minnesota Department of Human Services Background Study.
  • Valid Driver's License, full coverage insurance, reliable vehicle.
  • Willing to drive an approximately 30-mile radius, transporting clients into the community.
  • Must be very organized, detail-oriented, and a self-starter. Able to complete paperwork and follow instructions in a timely manner.
  • Must be computer literate, able to read and send emails from phone and computer. Able to use phone apps for mileage and timekeeping.
  • Possess problem-solving skills, able to multi-task, and follow through with all tasks given in the required time and to completion.
  • Must have excellent work ethic. Must be trustworthy, reliable, and willing to work entire shifts fulfilling all obligations.
  • Must be flexible and a team player. Willing to make changes in schedule to assist Supervisor when asked.
  • Must have compassion and want to go above and beyond to help individuals with disabilities to improve the quality of their life through supporting, teaching, advocating, and making their needs a priority.
  • Able to articulate well with family members, county workers, professionals, and clients.
  • Meet all DHS qualifications for ILS/IHS.
