Job summary

Seattle Indian Health Board is seeking a Care Coordinator/Clinical Case Manager to coordinate health and human services for Relatives (patients). You will serve as the point of contact for the care team and panel of Relatives, facilitating care links, scheduling, referrals, and system navigation.
