We help these individuals navigate chronic medical conditions, SUD, and Behavioral Health needs with a focus on prevention, integrative treatment, and recovery.

Holon Health’s initiatives develop, support, and maintain relationships with provider partners and community-based organizations to promote programs advocating long-term health and wellness for this complicated population. By acting as the first point of contact for the justice systems and programs with whom we partner, Holon Health provides timely and efficient brief interventions to better prepare patients to receive community-based services and engage in proactive, prosocial behaviors.

POSITION SUMMARY:

Holon Health is seeking a dedicated and passionate individual to join our team as a Certified Peer Recovery Specialist (Recovery Coach). As a Recovery Coach you will act as a vital member of our organization, providing support and guidance to individuals with Substance Use Disorders (SUD) who are on the path to recovery. You will play a vital role in bridging the gap between healthcare providers and the community, promoting health education, and empowering individuals to take charge of their well-being

SCHEDULE:

This role is remote but the individual must be certified and located in Indiana.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Establish a trusting and supportive relationship with individuals seeking care, serving as a role model and advocate.
  • Conduct individualized assessments to identify strengths, needs, and goals of individuals.
  • Collaborate with clients and Holon Health’s care team to develop personalized recovery plans, setting achievable objectives and milestones.
  • Connect clients with appropriate resources and community support services such as medical care, housing assistance, and employment/educational opportunities.
  • Conduct outreach and education programs on various health topics tailored to the specific needs of the community.
  • Provide culturally competent health information and resources.
  • Assist patients in navigating the healthcare system, including appointments, referrals, and understanding diagnoses.
  • Build trust and rapport with community members, fostering open communication and collaboration.
  • Identify and address social determinants of health that may be impacting individuals' health outcomes.
  • Stay up to date on current trends, research, and best practices in the field of addiction recovery.
  • Maintain HIPAA compliance at all times.
  • Other duties as assigned based on new initiatives for engagement.
Requirements

QUALIFICATIONS:

  • Must be a Certified Peer Recovery Specialist in the state of Indiana.
  • High school graduate or equivalent required; associate degree preferred.
  • Minimum of two years of experience working in a community health setting.
  • Experience working with individuals with SUD strongly preferred.
  • Strong understanding of health promotion principles and practices.
  • Outstanding interpersonal skills, with the ability to build rapport and establish trust with individuals.
  • Exceptional oral and written communication skills with the ability to actively listen and provide non-judgmental support.
  • Knowledge of addiction recovery principles, evidence-based practices, and resources available in the community.
  • Understanding of challenges and barriers individuals face during the recovery process and the ability to provide guidance and support.
  • Ability to maintain professional boundaries and confidentiality.
  • Ability to work in a dynamic and evolving environment.
