Job summary

Qualifications

• Current and unrestricted RN license in Indiana.
• Minimum 1 year nursing experience; home health and Medicaid preferred.
• CPR certification is required.
• Valid driver's license and reliable transportation.
• Familiarity with electronic documentation systems.

Responsibilities

• Provide skilled nursing services including wound care, medications, chronic disease management, and patient education.
• Ensure services are medically necessary and properly documented for Medicaid reimbursement.
• Coordinate care with physicians, case managers, and the interprofessional team.
• Educate patients and caregivers on disease processes, medications, and self-care strategies.
• Maintain accurate, timely, and compliant documentation in the EHR.
• Participate in Medicaid audits, quality assurance, and case conferences.
• Connect patients with community resources and support services.
