Job summary

FeldCare Connects is seeking a self-motivated Speech Therapist to provide home health services in the Alhambra area as an independent contractor. The role focuses on adults with speech, language, swallowing, and cognitive needs, developing and executing treatment plans, and monitoring progress.

The ideal candidate holds state licensure as an SLP, ASHA CCC-SLP, and is comfortable working independently in diverse home settings. Bilingual skills are a plus to support a varied patient population.

Qualifications

• Licensed and registered Speech Therapist with the state.
• Completion of an accredited Speech Therapist program.
• ASHA CCC-SLP credential preferred.
• Bilingual abilities are a plus.

Responsibilities

• Interpret, plan, establish, and execute a proper treatment program in accordance with the physician's referral.
• Record and report the patient's response to treatment and any alterations in condition.
• Instruct, teach, and monitor patients, their families, and other health team personnel regarding speech therapy procedures.
• Make recommendations based on home setting and individual needs of the patient.
• Keep a record of each patient visit and detailed notes in the medical record.
• Review responses to treatment and advise physician on continuation or discontinuation of therapy.
