Job summary

FeldCare Connects is seeking an enthusiastic Speech Therapist to provide quality home healthcare in Fontana and surrounding areas. This Independent Contractor role offers flexible scheduling and the autonomy to manage patient care across ages.

The clinician will perform one-on-one treatment, document progress in the EMR during visits, and determine plans of care while benefiting from administrative support and referral access through Cliniconnects.

Qualifications

• Current Speech Language Pathology license issued by the state Board of Speech-Language Pathology and Audiology.
• ASHA Certification (CCC) required.
• Completion of an accredited Speech Language Pathologist program.

Responsibilities

• Deliver one-on-one home healthcare to patients of all ages.
• Observe, record, and report patient response to treatment and changes in condition.
• Determine and implement the plan of care.
• Maintain documentation during visits using the EMR system.
