Ideal candidates are self-motivated, capable of working independently, and focused on helping patients reach their goals.

Qualifications

• Associate degree required with active BLS/CPR certification.
• Experience in home health care preferred.
• Must have valid Texas driver’s license and auto insurance.

Responsibilities

• Implement plan of care in consultation with physical therapist and care team.
• Communicate patient status and changes to physical therapist and care team.
• Educate patients and families on home safety and medical equipment safety.
• Be solution-driven, self-motivated, and inspiring to patients to reach goals.
• Work independently without direct supervision.
