Position Summary

Launch your services in Walmart stores as an Independent Optometrist leasing space inside our locations. You will play a key role in providing eye and vision care, along with general health and well-being services.

Autonomy

As an independent leaseholder, you can:

• Set your practice days and hours
• Manage your patient schedule
• Determine your fee structure
• Keep 100% of exam and professional fees

Optional support services available at fair market value include equipment/instrumentation and administrative or opt-tech support, if permitted by state regulations.

Minimum Qualifications

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional Requirements

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
