As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in patient care related to eye and vision health, as well as overall well-being.

Your autonomy as a leaseholder allows you to:

• Set your practice days and hours
• Manage your patient schedule
• Determine your fee schedule
• Keep 100% of exam and professional fees

Optional support services available for a fair market value include:

• Equipment and instrumentation
• Administrative and/or Opt-Tech support (if permitted by state)

Minimum qualifications for lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements include:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
