Position Summary...

What you'll do...

Launch your services in Walmart stores! As an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care related to eye and vision health, as well as general well-being.

As an independent leaseholder, you have the autonomy to:

1. Establish your days of practice
2. Establish your hours of operation
3. Control your patient schedule
4. Set your fee schedule
5. Keep 100% of exam/professional fees

For a fair market value, you can opt into:

1. Equipment/instrumentation
2. Administrative and/or Opt-Tech support services (if permitted by state)

Minimum Qualifications for a lease agreement:

1. Doctor of Optometry (OD) from an accredited optometry school
2. Valid state optometry license
3. Ophthalmologist (MD or DO) from an accredited school
4. Valid state ophthalmology license
5. General Liability Certificate
6. Professional Liability Insurance Certificate
7. W-9 form

ME & VA license agreements are with Gumberg Asset Management Company.
