Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being.

Autonomy includes:

• Establishing your practice days and hours
• Controlling your patient schedule
• Setting your fees
• Retaining 100% of exam/professional fees

Optional market value options include equipment and administrative/support services, if permitted by state.

Minimum qualifications:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements include:

• Ophthalmologist (MD or DO) with valid license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
