Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will lead patient care related to eye and vision health, as well as general well-being.

Your autonomy includes:

1. Establishing your practice days and hours
2. Controlling your patient schedule
3. Setting your fee schedule
4. Keeping 100% of exam/professional fees

For a fair market value, you may opt into:

• Equipment and instrumentation
• Administrative and/or Opt-Tech support services (if permitted by state)

Minimum qualifications include:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
