Position Summary:

As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in patient care related to eye and vision health, as well as general well-being. You will have the autonomy to:

1. Establish your practice days and hours
2. Control your patient schedule
3. Set your fee schedule
4. Keep 100% of exam/professional fees

For a fair market value, you can opt into:

1. Equipment and instrumentation
2. Administrative and/or Opt-Tech support services (if allowed by state)

Minimum Qualifications:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school (if applicable)
• Valid state ophthalmology license (if applicable)
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
