This position offers the opportunity to launch your services within Walmart stores as an independent lease holder. You will play a key role in patient eye and vision care, as well as general health and well-being.

As an independent lease holder, you have the autonomy to:

1. Establish your days of practice
2. Set your hours of operation
3. Control your patient schedule
4. Set your fee schedule
5. Keep 100% of exam/professional fees

Optional support services available for a fair market value include:

• Equipment/instrumentation
• Administrative and/or Opt-Tech support services (if permitted by state)

Minimum Qualifications for a lease agreement:

• Doctor of Optometry (OD) from an accredited school
• Valid state optometry license

Additional requirements:

• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9 form
