Minimum qualifications for lease agreement include:

1. Doctor of Optometry (OD) from an accredited school
2. Valid state optometry license
3. Ophthalmologist (MD or DO) from an accredited school (if applicable)
4. Valid state ophthalmology license (if applicable)
5. General Liability Certificate
6. Professional Liability Insurance Certificate
7. W-9 form
