Job summary

Walmart is seeking an Independent Optometrist to lease space inside Sam’s Club in Muncy, PA. You will lead eye and vision care along with general health and well-being, with autonomy to set your days, hours, and patient schedule.

You will have control over your fee schedule and keep 100% of exam and professional fees. ME & VA license agreements are handled with Gumberg Asset Management Company; W-9 is required.

Qualifications

• OD degree from an accredited optometry program.
• Active, valid optometry license in the state of practice.
• MD/DO as ophthalmologist qualifies for related licensing.
• Active ophthalmology license in the state of practice.
• General liability certificate.
• Professional liability insurance certificate.
• W-9 form required.

Responsibilities

• Lease an independent optometrist space inside Sam’s Club and lead patient care in eye and vision health.
• Establish your practice days and hours of operation and set your patient schedule.
• Set your fee schedule and retain 100% of exam and professional fees.
• Opt-in to equipment/instrumentation and support services where allowed by state law.

Education

Doctor of Optometry (OD) from an accredited optometry school

Valid state optometry license

Ophthalmologist (MD or DO) from an accredited school

Valid state ophthalmology license

General Liability Certificate

Professional Liability Insurance Certificate
