Position Summary

You will launch your services in Sam’s Clubs. As an independent lease holder, you will take a leading role in patient care related to eye and vision care as well as general health and well‑being, while maintaining full operational autonomy.

Responsibilities

• Lead patient care involving eye and vision examinations, refractions, and general health assessments.
• Manage your own schedule, hours of operation, and fee structure.
• Maintain compliance with all state optometry regulations and report requirements.

Lease Opportunities

• Establish your days of practice.

• Keep 100% of exam/professional fees.

Minimum Qualifications for a lease agreement

• Doctor of Optometry (OD) from an accredited optometry school.

• Valid state ophthalmology license (required for ophthalmologist role).
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W‑9 form.
