Job summary

A leading retail membership club in Slidell is seeking an Independent Optometrist to lease space and provide patient care related to eye and vision health. You will have the autonomy to set your practice schedule and keep all professional fees. Ideal candidates will hold a Doctor of Optometry (OD) and a valid state optometry license.

Qualifications

• Doctor of Optometry (OD) is required.
• Valid state optometry license is mandatory.
• Experience in a clinical setting is preferred.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 forms.

Responsibilities

• Take a leading role in patient care.
• Establish days and hours of practice.
• Set patient and fee schedules.
• Set your fee schedule.
• Keep 100% of exam/professional fees.
• Option to include Advanced Equipment/instrumentation and administrative/Opt-Tech support if state allows.

Position Summary: Independent Optometrist leasing space inside Sam’s Club, taking a leading role in patient care with respect to eye and vision care as well as general health and well-being.

Launch your services in Sam’s Clubs and enjoy the autonomy of an independent lease holder, including setting your own days of practice, hours of operation, patient schedule, and fee schedule, with the option to keep 100% of exam/professional fees. For a fair market value, you can opt into equipment/instrumentation and administrative and/or Opt-Tech support services (if state allows).

What you’ll do

As an Independent Optometrist leasing space inside our stores you will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing. You have the autonomy to:

• Establish your days of practice
• Establish your hours of operation
• Control your patient schedule
• Set your fee schedule
• Keep 100% of exam/professional fees

For a fair market value, you can opt into:

• Administrative and/or Opt-Tech support services if state allows
