Primary Location: 2600 PLANK ROAD COMMONS, ALTOONA, PA 16601-9310, United States of America. Walmart invites an Independent Optometrist leasing space inside Walmart stores to lead eye/vision care and general health services.

You will establish your practice days, hours, and patient schedule while setting your own fee schedule and keeping 100% of exam fees. Optional equipment and support may be available where state law allows.

Qualifications

• OD from an accredited optometry school
• Valid state optometry license
• Ophthalmologist MD/DO from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate

Responsibilities

• Launch your optometry services in Walmart stores and lead patient care for eye/vision and general health
• Establish days, hours, and control your patient schedule as an independent lease holder
• Set your fee schedule and retain 100% of exam/professional fees
• Optionally utilize equipment and administrative/Opt-Tech support (if state allows)

Education

Doctor of Optometry (OD) from an accredited optometry school

Valid state optometry license

Ophthalmologist MD or DO from an accredited school

Valid state ophthalmology license

General Liability Certificate

Professional Liability Insurance Certificate
