Job summary

Walmart is seeking an Independent Optometrist to lease space inside a Sam’s Club in Sugar Land, TX. You will lead eye care services and maintain autonomy over your practice days, hours, patient scheduling, and fee schedules.

As the lease holder, you may access equipment or administrative support if allowed by state regulations, while you manage your own schedule and fees to reflect fair market value.

Qualifications

• Minimum Qualifications for a lease agreement: Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9

Responsibilities

• Lease position within a Sam’s Club optometry space with autonomy to set practice days and hours
• Establish your hours of operation and patient schedule
• Set your fee schedule and keep 100% of exam/professional fees
• Arrange optional equipment/administrative support if state allows

Education

OD degree from an accredited optometry school

Valid state optometry license

MD/DO ophthalmologist degree

Valid state ophthalmology license

General Liability Certificate

Professional Liability Insurance Certificate

W-9 form
