Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.

Responsibilities

• Launch your services in Sam's Club and lead patient care in eye, vision, and general health.
• Establish your days of practice and hours of operation, control your patient schedule, and set your fee schedule.
