Entering this lease arrangement requires active OD and state ophthalmology licensure, liability certificates, and eligibility to operate under Walmart’s health services framework where permitted by state law.

Qualifications

• OD degree from an accredited optometry school.
• Active state optometry license.
• MD/DO ophthalmology license with valid state license.
• General liability certificate.
• Professional liability insurance certificate.

Responsibilities

• Lead patient care in eye and vision health within Walmart stores.
• Set your days and hours of operation for your practice.
• Control your patient schedule and fee schedule.
• Keep 100% of exam/professional fees.
• Optionally access equipment, administrative and/or opt-tech support services if state allows.
