You will operate as an independent provider within the club, enjoying autonomy over your schedule and patient flow while maintaining professional standards and compliance.

Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD/DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form.

Responsibilities

• Own your days of practice and hours of operation.
• Control your patient schedule and set your fee schedule.
• Keep 100% of exam/professional fees.
• Leasing inside Sam’s Club as an Independent Optometrist.
• Provide eye and vision care and general health guidance.

Education

Doctor of Optometry (OD) from an accredited optometry school

Valid state optometry license

Ophthalmologist (MD or DO) from an accredited school

Valid state ophthalmology license

General Liability Certificate
