Curana Health in the United States is seeking licensed Nurse Practitioners or Physician Assistants to lead primary care for seniors in skilled nursing and senior living communities. You will deliver proactive, relationship-based care in home-like settings with support from physicians and a tech-enabled care model.

You'll have autonomy, flexible scheduling, and opportunities for professional growth while contributing to reduced hospitalizations and improved outcomes.

Formación

• Must be licensed as a Nurse Practitioner or Physician Assistant in the operating state.
• Master's degree required in nursing or PA studies with board certification.
• Strong leadership, interpersonal, and problem-solving skills are preferred.

Responsabilidades

• Deliver comprehensive primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose, treat, and manage acute and chronic conditions with emphasis on preventative care.
• Build and maintain meaningful patient relationships and coordinate care with the care team.
