Job summary

Lorain County Health & Dentistry is seeking an Independent Mental Health Therapist (LISW/LPCC) to join our Behavioral Health team on site in Lorain, OH. You will provide mental health and substance use assessments, crisis intervention, and short-term counseling to primary care patients.

Requirements include a Master’s in Social Work or Counseling and an active LISW/LPCC license in Ohio; two years of relevant experience; bilingual Spanish is a plus.

Qualifications

• Master’s Degree in Social Work or Counseling.
• Must hold a valid Ohio license as LISW or LPCC.
• Minimum of two years in a mental health setting; primary care integration preferred.
• Experience with substance use assessment/treatment preferred.
• Bilingual Spanish speaking is a plus.

Responsibilities

• Evaluate and assess mental health and substance use symptoms in patients.
• Intervene in crisis situations during business hours.
• Provide brief assessments to inform care plans.
• Define targeted treatment goals with patients.
• Provide short‑term counseling to Lorain County Health & Dentistry patients.
• Identify patients needing intensive behavioral health care and connect to community resources.
• Ensure plans are personalized, age specific, and culturally appropriate.
• Provide education and support using trauma‑informed approaches.
• Develop positive patient relationships and promote health throughout the facility.
• Accurately document all patient contacts in the medical record.
• Collaborate with medical and psychiatric providers for integrated care.
• Participate in Peer Review to ensure quality of BH services.
• Comply with HIPAA and 42 CFR guidelines.
