Job summary

BAYADA Home Health Care is seeking a full-time Licensed Practical Nurse (LPN) to join our home health visit team in the Denver area. Travel within the coverage area is required; prior home health experience is strongly preferred.

You’ll be part of a multi-disciplinary team delivering skilled nursing and rehabilitative care to help clients remain at home, with a focus on compassionate, quality care and reliability.

Qualifications

• Current Colorado LPN license in good standing.
• Graduate from an accredited nursing program.
• At least one year of hands-on medical experience with adult/geriatric populations.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify changing conditions.
• Provide skilled nursing and rehabilitative care in patients' homes.
• Triage and manage urgent clinical situations independently.
• Reconcile medications and provide patient teaching and safety monitoring.
• Document wound assessments, care, and progress in the chart.
