Creating a Legacy, Inc. is seeking a dedicated Personal Support Instructor (PSI) to provide direct care and daily living support in Long Beach, CA. You will help individuals follow ISP plans and promote independence with structured routines.

You will assist with ADLs, document progress, support community participation, and ensure health and safety. A California driver’s license and reliable transportation are required.

Qualificações

• High School Diploma or GED is required.
• Driver’s License and Insurance: valid CA license, clean driving record, and auto insurance.
• Background clearance required.
• 1–4 years of direct support experience preferred.
• Documentation skills for daily notes and MAR.
• Time management and professional communication.
• Ability to work as part of a team.

Responsabilidades

• Assist individuals with personal care and ADLs.
• Implement ISP goals and structured routines.
• Encourage community involvement and social activities.
• Maintain accurate daily records and MAR.
• Administer medications as authorized and trained.
• Maintain health and safety in the living environment.
• Attend trainings and staff meetings.
• Respond to emergencies per procedures.
• Report incidents and concerns to supervisors.
