Qualifications

• High School diploma or GED required.
• 1 year of supervised work experience in public health or human services.
• 2 years' experience with individuals with developmental disabilities.
• CNA or Medical Assistant certification may qualify for special entry rate.

Responsibilities

• Manage and supervise residents' active treatment program per individual plans.
• Follow activities to meet objectives and provide services from the plan.
• Initiate learning skills programs (reading, money management, cooking, grooming).
• Complete forms and reports, including progress notes.
• Provide care for daily living skills and home management, cooking, cleaning, and socialization.
• Maintain a safe and healthy environment for residents.
• Offer community inclusion activities for residents.
• Monitor and remind residents to take medications as needed.
• Document resident behavior and needs for shift continuity.
• Provide transportation to clients for Pathfinder business.
