Job summary

Qualifications

• Education: High School Diploma.
• Experience with seriously mentally ill or similar population.
• Strong judgment and customer service skills.
• Ability to manage time and stay organized.
• Ability to de-escalate crisis situations.
• Ability to work with others and independently.

Responsibilities

• Provide case management to residents.
• Implement individual service plans to help rehabilitation goals.
• Assist residents with daily living skills (grooming, meals, budgeting, etc.).
• Document services and interactions in case records.
• Engage residents in regular goal discussions and plan evaluations.
• Provide counseling support as needed.
• Coordinate services with medical, legal, and community resources.
• Monitor incidents and minimize risk; notify the Manager when needed.
