Fox Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in patients' homes and senior living communities in Wilkes-Barre, PA. The clinician will assess, treat, and document using our Geriatric House Calls model, with telehealth options to enhance access and continuity of care.

You will build lasting patient relationships, schedule your own appointments, and collaborate with an interdisciplinary team to improve mobility and independence for older adults.

Formación

• Licensed Physical Therapist eligible to practice in PA or eligible to obtain licensure.
• Current CPR certification and basic computer/EMR proficiency.
• Experience delivering one-on-one geriatric PT is preferred.

Responsabilidades

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Develop personalized plans of care and deliver evidence-based treatment.
• Deliver sessions that improve mobility, independence and function.
• Document in EMR in compliance with regulations.
• Collaborate with PT/OT/SLP and Fitness Specialists as part of the team.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
