FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier home health care for adults of all ages in the Antioch area. The role involves assessment, planning, execution of treatments, and ongoing progress monitoring in a home-health setting.

The ideal candidate holds a state PT license, is capable of working independently, and can communicate effectively. Bilingual skills are a plus, and flexibility is emphasized to balance patient needs and schedule.

Qualifications

• Must be licensed as a Physical Therapist in the state.
• Enthusiasm for working with patients of all ages.
• Bilingual a plus.

Responsibilities

• Assess patient care needs and determine a treatment plan.
• Deliver interventions to improve movement, reduce pain, restore function and prevent disability.
• Monitor patient progress and adjust care as needed.
