Brooks Home Health in Ocala, FL is seeking a registered nurse to provide comprehensive care planning and management for patients in collaboration with a multidisciplinary team. You will initiate individualized nursing plans within 24 hours of admission, perform medication reconciliation, document outcomes, educate patients and families, and coordinate care across shifts.

Qualifications

• Registered Nurse license (FL) required.
• BLS certification required.
• Degrees in Nursing from an accredited school.

Responsibilities

• Identify actual/potential nursing/clinical problems and document findings.
• Initiate and establish individualized nursing plan of care within 24 hours.
• Develop patient-centered goals with families and rehab team.
• Document changes and outcomes; adjust care plan as needed.
