Job summary

Inspired Home Care, a 5-Star ACHC-accredited agency, seeks PRN and Part Time Home Health Nurses to manage their own caseload in Elmhurst, IL. You will visit patients at home, assess health status, document findings and provide compassionate care tailored to individual needs.

Independent work is essential, with strong time management and clear communication. Travel to multiple patients per day is required, collaborating with physicians and therapists to optimize care plans.

Qualifications

State RN license required.
1 year of experience in in-home care.
Excellent communication and time management skills.
Compassionate and friendly demeanor.
Ability to travel to multiple patients per day.
Preferred knowledge of WellSky Kinnser.

Responsibilities

Assess patients' condition during every visit and chart observations.
Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy.
Dress or re-dress open wounds and assess healing progress.
Educate patients and families on home health care strategies and procedures.
Coordinate with therapists, physicians and others involved in the care plan.
Make recommendations for devices or tools to improve quality of life.
Listen to the patient and respond to concerns or requests.
