Bradford, VT-based Visiting Nurse and Hospice for VT & NH is seeking a licensed Physical Therapist to provide in-home evaluations and treatment for patients under physician orders. You will collaborate with the care team, supervise PTA, and develop individualized treatment plans.

Formación

• Must possess an active PT license or be license eligible in both VT and NH.

Responsabilidades

• Clinical assessment and treatment of patients referred by a licensed physician.
• Coordinate and communicate treatment activities with health care team members and the patient.
• Supervise PTA and contribute to the development of new staff and students.
• Evaluate patient needs and develop appropriate treatment plans.
• Provide care based on physician plan, policy, and standards of care.
• Instruct family members and caregivers in PT techniques.
• Coordinate progress with other caregivers and refer to other professionals when appropriate.
