CenterWell Home Health is seeking a licensed practical nurse (LPN) to provide skilled, one-on-one care in patients' homes under the supervision of an RN and Clinical Manager.

You'll implement individualized care plans, educate patients and families on disease management, and document visits, interventions, and outcomes while coordinating with physicians and the healthcare team to support recovery and independence.

Qualifications

• Current nursing license in practicing state.
• Valid CPR certification.
• One year experience as an LPN/LVN in a clinical setting.

Responsabilités

• Provide skilled nursing care to patients, in patients' homes.
• Implement individualized care plans with patient/family and healthcare team.
• Educate patients and families on disease management and self-care techniques.
• Maintain accurate records of patient care and visit documentation.
• Coordinate with physicians and clinical managers and participate in discharge planning.

Connaissances

Current nursing license

Valid driver's license

CPR certification

One year experience LPN/LVN
