Job summary

Guardian Angel Home Care Inc. seeks a compassionate Licensed Practical Nurse (LPN) to join our home health team. You will provide patient-centered care in patients’ homes, working with an interdisciplinary team to support autonomy and meaningful patient relationships.

Qualifications

• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong clinical, communication, and documentation skills.
• Ability to work independently in a home health setting.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of the RN and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered within the LPN scope.
• Document all visits and care provided in HCHB.
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and plan of care.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal outcomes.
