Zusammenfassung

Guardian Angel Home Care Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team. You will provide patient-centered nursing care in patients' homes and collaborate with a supportive interdisciplinary team.

This role offers autonomy, flexible scheduling, and meaningful connections with patients while contributing to high-quality care in a home health setting.

Qualifikationen

Graduate of an accredited practical nursing program.
Current BLS/CPR Certification.
Valid driver's license, reliable transportation and active auto insurance.
Strong clinical, communication, and documentation skills.
Ability to work independently in a home health setting.

Aufgaben

Provide skilled nursing care in patients' homes under the direction of the RN and physician.
Monitor patient condition, vital signs, and overall health status.
Administer medications and treatments as ordered and within the LPN scope of practice.
Document all visits and care provided in Homecare Homebase (HCHB).
Communicate changes in patient condition to the supervising RN and physician.
Educate patients and families on medications, disease management, and plan of care.
Ensure compliance with federal, state, and agency regulations.
Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
