Avail Health & Behavioral Solutions, LLC in Jacksonville, FL, seeks a Direct Care Provider to join our registry as an independent contractor. You will deliver one-on-one community support for a 30-year-old male on the autism spectrum, acting as a companion and mentor to help practice social skills in real-world settings.

The role starts with about 5 hours per week, can grow to 25 hours as the client acclimates, with schedules Mon-Fri and flexible hours, ideal for experienced caregivers or

Qualifications

• High School Diploma or GED required.
• Valid driver’s license and reliable vehicle for professional use.
• Ability to pass a Level II Background Screening.
• Experience supporting individuals with disabilities or behavioral health needs.
• Demonstrated ability to manage client care and safety with limited supervision.
• Personal smartphone required for app-based scheduling and client communication.
• CPR/First Aid Certification required within 30 days of onboarding.
• APD Training Certificates required within 30 days of onboarding.
• CNA, BLS, HHA, or DSP certifications preferred (not required).
