• Independently manages a panel of patients with acute and chronic conditions with emphasis on prevention using evidence-based practice
• Develops and implements individualized plans to achieve population health goals
• Leads and collaborates with Primary Care Team and interdisciplinary members
• Orders and interprets diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals to specialty services
• Provides education to patients and families
