Community Connections for Independent Living, LLC invites applicants for a Rehabilitation Coach in Grand Rapids, MI. The role supports residents in achieving independence within five life domains and implements Master Treatment Plans in home or community settings.

Responsibilities include documentation, communication with therapists, organizing recreational activities, and transportation to appointments. Requires an associate degree, Michigan license, BLS/First Aid, a background check, and CBIS

Formación

• Associate degree from an accredited institution.
• Must possess a valid Michigan Operator's License and meet driving criteria.
• BLS/First Aid Certification through the American Heart Association.
• Criminal background check required.
• Willingness to complete CBIS training after employment.

Responsabilidades

• Promotes residents’/clients’ independence within 5 Life Domains (ADLs, Cognitive Abilities, Social Participation, Education and Work).
• Provides assistance and training to residents/clients to achieve goals of Master Treatment Plan in private home or community-based settings.
• Completes all required documentation per Community Connections’ protocols for each resident/client.
• Communicates changes in a resident’s/client’s condition to lead Occupational Therapist and Director of Clinical Services.
• Assists in planning and implementing recreational outings related to residents’ interests and avocations.
• Transports residents/clients to appointments and community/recreational outings using organization vehicles.
• Participates in assessment of progress toward goals within Individualized Rehabilitation Plan (IRP).
• Performs daily physical checks and phone checks for each resident/client as assigned.
• Participates in staff meetings and team conferences.
• Maintains professional, ethical, timely performance.
• Ensures a safe and therapeutic environment for residents/clients.
• Adheres to policies as outlined in the Employee Manual.
