Job summary

Responsibilities

• Conduct in-depth assessments and create individualized treatment plans.
• Provide psychotherapy to address mental health needs and psychological aspects of medical issues.
• Support through case management: help patients follow through on their treatment plans and connect with resources to promote recovery and well-being.
• Maintain detailed and timely documentation in our EHR system.
