Job summary

Harboroaks is seeking a licensed Master’s Social Worker to plan and coordinate individualized therapeutic support for patients with behavioral health needs in a clinical setting. The role emphasizes psychosocial assessments, treatment planning, crisis intervention, and collaboration with a multidisciplinary team.

The ideal candidate holds an MSW from an accredited program, LMSW/LCSW/LSW licensure, CPR and de-escalation certification, and at least two years’ experience.

Qualifications

• Master's degree in Social Work from an accredited program.
• Two or more years’ experience with patient populations preferred.
• Licensure (LMSW/LCSW/LSW) as required by state facility.
• CPR and de-escalation/restraint certification required.

Responsibilities

• Conduct psychosocial assessment and develop treatment plans.
• Provide crisis intervention and support families.
• Participate in multidisciplinary treatment team meetings.
• Coordinate patient cases and discharge planning.
• Facilitate education, process groups, and referrals.
• Maintain accurate documentation and patient records.
