Job summary

Harboroaks is seeking a PRN Social Worker to plan, direct, and coordinate individualized and group therapeutic services for patients with behavioral problems and psychiatric disorders. Qualifications

• Master's degree in Social Work from an accredited college or university required.
• Three or more years' related experience working with comparative patient population required.

Responsibilities

• Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.
• Coordinate patient cases and anticipate needs related to illness, hospitalization, finances, education and home/community services.
• Provide crisis intervention and assist families in understanding implications and complexities of a life situation.
• Participate in multidisciplinary treatment team meetings to discuss psychosocial assessment, treatment planning and discharge planning.
• Promote recovery, symptom reduction and the achievement of the highest level of social functioning.
