Job summary

Qualifications

• High school diploma or GED or equivalent combination of education and experience.
• Current LPN licensure in the state of employment.
• IV certification required at time of hire; timelines given.
• Active BLS for healthcare professionals.
• Basic English communication required.
• Flexible schedule to meet patient needs.
• Ability to use equipment with visual and auditory cues.
• Ability to meet physical demands and lifting requirements.

Responsibilities

• Under RN supervision, perform nursing interventions to maximize patient comfort and quality of life.
• Provide nursing tasks including injections, dressing changes, catheter care as per plan of care.
• Observe patients and report changes to RN, including symptoms and adverse reactions.
• Document care per policy in medical records.
• May be required to report to alternate locations during emergencies to ensure continuity of care.
• Perform other duties as assigned.
• Cross-train to Hospice House roles when needed.
