Job summary

Evolve Therapy Services seeks a Speech Language Pathologist to join our in-house Skilled Nursing Facility team in sunny Florida. This is a 6-month contract-to-permanent opportunity offering growth and relocation assistance as needed.

Responsibilities include planning, directing, and evaluating speech language pathology services, with emphasis on CMS-compliant documentation and FEES/MBS oversight. Active state licensure and CCC are required, with opportunities for professional development.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency is required.
• Active SLP license in states of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and collect data for treatment planning and implementation.
• Develop treatment plans with short-term and long-term goals.
• Instruct patients, families, and staff in speech-language procedures for carryover of remediation.
• Complete compliant CMS-mandated documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to assess resident needs.
