Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist to provide in-house therapy at a partner Skilled Nursing Facility in Burlington, WI. You will evaluate residents, develop plans, and work with staff to meet care standards.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC certification obtained (ASHA).
• Active SLP license in state(s) of practice.
• Commitment to ongoing continuing education.

Responsibilities

• Provide quality speech therapy services in an in-house SNF setting.
• Evaluate data for treatment planning and implementation.
• Develop a treatment plan with short- and long-term goals.
• Educate patient, family, and staff for carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site coordination.
• Participate in screening to meet resident needs timely.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency (CCC)

Active SLP license in state(s) of practice

Continuing education to maintain licensure
