Job summary

Evolve Therapy Services is hiring a Speech Language Pathologist to work in an in-house Skilled Nursing Facility partner in Burlington, WI. This full-time or PRN position focuses on planning, directing, and delivering Speech Language Pathology services to residents to ensure high-quality care.

The role requires a Master’s in Speech Language Pathology, CCC-SLP, and an active state license, with CMS documentation responsibilities and FEES/MBS protocol oversight.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes a target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Continuing education to maintain licensure
