Evolve Therapy Services seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Massena, NY. The role involves evaluating residents, designing treatment plans, and delivering direct therapy to meet CMS standards.

Requirements include a Master’s in Speech-Language Pathology, CCC, and an active SLP license. Ongoing CEU participation is expected to maintain licensure and clinical excellence.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• CCC credential required to practice as a licensed SLP.
• Active licensure in the state(s) of practice is mandatory.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals per standards.
• Evaluate and collect data for treatment planning and progress.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff on speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening to identify residents' needs promptly.
